    Job Description
    General Statement of Duties:  The Services Coordinator is responsible for responding to service inquiries, initiating intakes, discharges, and referral coordination within the various programs, as well as other agencies.
    Primary Duties:
    • Identify/triage the needs of client/family; coordinate and assure linkages to services in a timely manner.
    • Make all Outpatient Behavioral Health appointments in calendar and assure proper notification to all parties. Schedule Childhaven vehicles and meeting rooms in an efficient manner that meets the needs of both client and Childhaven staff.
    • Conduct a face to face intake/discharge with the client and guardian, ensuring completion & signatures.
    • Ensure client files/documents are delivered to the proper staff or outside agency in a timely manner.
    • Track all timelines to assure timely access to services and aftercare services are utilized.
    • Complete quality assurance audits of specified number of client files and submit report to program director quarterly.
    • Complete regular Medicaid eligibility checks/Program Funding Registration/identification for VOCA, Family Support and OPBH clients.
    • Create/maintain OPBH client rosters/attendance/no show spreadsheet, billing and payroll crosscheck
    • Assure the completion and updating of Childhaven procedures for Services Coordinator duties.
    • Inventory/Order supplies as needed.  Ensure that supply closet is maintained with supplies and orderly.
    • Facilitates Monthly Utilization Review meeting and completes sign in sheet and minutes.
    • Assume other related tasks as assigned by your supervisor.
    Job Requirements:
    • Demonstrated ability to deal effectively with children/youth and their families in a customer friendly manner.
    • Current CPR First Aid and Crisis Intervention Prevention (CPI) within 90 days of hire.
    • Flexibility and ability to work in crisis situations.
    • Ability to manage multiple tasks at once.
    Job Knowledge:
    • Working knowledge of human development, mental health diagnosis, and substance abuse.
    • Excellent oral/written communication/organizational skills, adept at in working with people in a culturally diverse environment, fluency in Navajo or Spanish desired.
    • Familiarity with community services, social services delivery systems, medical office experience a plus.
    • Understanding strict confidentiality requirements and adherence to HIPAA regulations.
    AA degree in a human services or related field with 2 yrs. experience or H.S. Diploma or GED, plus 3 yrs. related experience.
